OCD Treatment in Rawalpindi β€” Waswasay Ka Ilaj

Obsessive Compulsive Disorder (OCD) β€” known in Urdu as "waswasay" β€” is a condition that causes unwanted, intrusive thoughts (obsessions) and repetitive behaviours or mental acts (compulsions) performed to reduce anxiety. OCD can be severely disabling, consuming hours of a person's day, but it responds very well to the right treatment.

What Is OCD?

OCD is characterised by a cycle: an intrusive thought creates intense anxiety, the person performs a compulsion to relieve the anxiety, relief is temporary, and the cycle repeats. Over time, the compulsions become more elaborate and time-consuming.

Common OCD Presentations in Pakistan

Contamination OCD

Fear of germs, dirt, or contamination β€” leading to excessive hand washing, cleaning, or avoidance of certain objects or places.

Religious OCD (Waswasay)

Intrusive thoughts about religion, blasphemy, or sinfulness that cause intense guilt and distress. Very common in Pakistan β€” often mistaken for a spiritual problem rather than a medical one. Performing extra prayers or religious rituals to neutralise these thoughts is a compulsion, not genuine faith.

Checking OCD

Repeatedly checking locks, gas, appliances, or other things β€” driven by fear that something terrible will happen if not checked.

Harm OCD

Intrusive thoughts about accidentally or intentionally harming oneself or others. People with harm OCD are not dangerous β€” they are horrified by these thoughts and go to great lengths to avoid acting on them.

Important: Religious OCD (waswasay) is a medical condition, not a spiritual failing. Many religious scholars in Pakistan have clarified that OCD-related intrusive thoughts are not sinful. Seeking psychiatric treatment is not only acceptable but necessary.

Exposure and Response Prevention (ERP)

ERP is the gold-standard psychological treatment for OCD. It involves gradually exposing the patient to situations that trigger obsessions while resisting the compulsion to respond. This breaks the OCD cycle and rewires the brain's response to intrusive thoughts. It sounds difficult, but with professional guidance it is highly effective.

Medication

SSRIs at higher doses than used for depression are effective for OCD. Medication reduces the intensity of obsessions, making ERP therapy easier to engage with. A combination of ERP and medication produces the best results.
