Bipolar Disorder: Diagnosis & Treatment in Pakistan

Bipolar disorder is one of the most misunderstood and misdiagnosed mental health conditions in Pakistan. Many patients spend years receiving treatment for depression alone before the correct diagnosis is made. With proper diagnosis and treatment, people with bipolar disorder can live full, stable, and productive lives.

What Is Bipolar Disorder?

Bipolar disorder is a mood disorder characterised by extreme shifts between depressive episodes (low mood, low energy, hopelessness) and manic or hypomanic episodes (elevated mood, high energy, reduced need for sleep, impulsive behaviour). These mood episodes can last days, weeks, or months.

Types of Bipolar Disorder

Bipolar I

At least one full manic episode, which may be severe enough to require hospitalisation. Depressive episodes are also common.

Bipolar II

Recurrent depressive episodes with at least one hypomanic episode (less severe than full mania). Often mistaken for depression alone.

Cyclothymia

Milder mood fluctuations that do not meet the full criteria for bipolar I or II but still significantly affect daily life.

Signs of a Manic Episode

Key fact: Bipolar disorder is frequently misdiagnosed as depression in Pakistan because patients often seek help during depressive episodes, not manic ones. Always tell your psychiatrist about periods of unusually high energy, reduced sleep, or impulsive behaviour.

Treatment for Bipolar Disorder

Mood Stabilisers

Medications such as lithium, valproate, and lamotrigine are the cornerstone of bipolar treatment. They reduce the frequency and severity of both manic and depressive episodes. These medications require regular monitoring but are safe and highly effective.

Psychotherapy

Psychoeducation (learning about the condition), CBT, and family therapy all play important roles in managing bipolar disorder. Therapy helps patients recognise early warning signs of episodes and develop strategies to prevent escalation.
