Child Mental Health

Child & Adolescent Psychiatry in Pakistan

By Dr. Mahmood Ali Khan, Associate Professor of Psychiatry  |  Safari Hospital, Rawalpindi  |  July 2026

Children and teenagers face unique mental health challenges that require specialist care. As a child and adolescent psychiatrist with over 20 years of experience, I understand that early intervention is critical β€” mental health conditions that go untreated in childhood can have lifelong consequences.

Why Child Mental Health Matters

Half of all lifetime mental health conditions begin before the age of 14. In Pakistan, where awareness of child mental health is still developing, many children suffer without proper diagnosis or treatment. Early identification and treatment can transform a child's future.

Key fact: Children cannot always express their emotional distress in words. Behavioural changes, school problems, physical complaints, and withdrawal are often how mental health issues present in children.

Common Child Mental Health Conditions

ADHD (Attention Deficit Hyperactivity Disorder)

ADHD is one of the most common childhood mental health conditions. Children with ADHD struggle with attention, impulsivity, and hyperactivity. Without treatment, ADHD can lead to academic failure, low self-esteem, and relationship difficulties.

Signs to watch for: Inability to sit still, constant distraction, forgetting instructions, talking excessively, difficulty completing tasks.

Childhood Anxiety

Anxiety in children is often overlooked or mistaken for shyness. It can manifest as school refusal, excessive clinginess, sleep problems, stomach aches before school, and extreme worry about everyday situations.

Childhood Depression

Children can suffer from depression just like adults, but it often looks different. Watch for: persistent irritability, withdrawal from friends and activities, changes in sleep or appetite, declining school performance, and expressions of hopelessness.

Autism Spectrum Disorder (ASD)

Autism affects social communication and behaviour. Early diagnosis and intervention β€” ideally before age 5 β€” significantly improves outcomes. Signs include delayed speech, lack of eye contact, repetitive behaviours, and difficulty with social interaction.

Conduct Disorder & Oppositional Defiant Disorder

Some children exhibit persistent patterns of defiance, aggression, or rule-breaking that go beyond normal behaviour. These conditions respond well to early intervention combining behavioural therapy and family support.

Treatment Approach for Children

Family Involvement

Parents and family members are central to a child's treatment. Dr. Mahmood works closely with families to understand the child's environment, relationships, and stressors, and to equip parents with strategies to support their child at home.

Child-Friendly Therapy

Play therapy, art therapy, and child-adapted CBT are used to help younger children process their emotions and develop coping skills in age-appropriate ways.

School Coordination

When appropriate, Dr. Mahmood can provide guidance to schools on how to support a child with mental health needs in the classroom.

Medication When Necessary

Medication for children is prescribed with great caution, only when clearly indicated, at the lowest effective dose, and with close monitoring. It is never the first-line treatment for mild conditions.
